脊骨神經醫學養成教育

一、美國脊骨神經醫學院課程
1. 大學預醫先修課程 ( Pre-medical programs )：

修業 3-4 年，所修課程包括生物、物理、化學、有機化學、心理學、美國通識課程 ( General Education ) 等約 90 學分，取得學士學位。

2.脊骨神經科醫學院 ( Chiropractic college )：

修業十學期，共修約 260 學分，相當於臺灣學士後醫學系的體制，而以僅存之高雄醫學院學士後醫學系來說，其修業５年，只修 200 學分，脊骨神經科學院多了近三分之一的學分。所修課程包括：前二年之基礎醫學課程─解剖學、生理學、生物化學、組織學、胚胎學、免疫學、微生物學、病理學、遺傳學及營養學等。後二年之臨床醫學─骨科學、皮膚科學、婦產科學、小兒科學、老人醫學、神經學、X 光放射線學、精神科學、臨床檢驗學、藥理學、公共衛生學、脊椎生物力學、動力學、脊椎矯正、物理治療、復健醫學等。最後至少須有一年以上之醫院實習，才完成脊骨神經科醫學博士學位 ( Doctoral Degree in Chiropractic ).

3.脊骨神經科次專科訓練：

脊骨神經科醫師在取得一般醫師執業執照後，可再繼續攻讀三年次專科學位，取得專科醫師資格。例如：
骨科專科醫師 ( Diplomate of the American Board of Chiropractic Orthopedics, DABCO )
神經科專科醫師 ( Diplomate of the American Chiropractic Board of Neurology, DABCN )
放射專科醫師 ( Diplomate of the American Chiropractic Board of Radiology, DACBR )
營養專科醫師 ( Diplomate of the American Chiropractic Board of Nutrition, DACBN )
運動專科醫師 ( Diplomate of the American Chiropractic Board of Sports Physicians, DACBSP )
內科專科醫師 ( Diplomate of the American Chiropractic Board of Internist, DABCI )
小兒專科醫師 ( Diplomate of the American Chiropractic Board of Pediatric, DABCP )
足科專科醫師 ( Certified Chiropractic Foot Care, CCFC )
脊椎側彎專科醫師 ( Board Certified in Scoliosis )

二、執照考試方式

目前全美國各州均已立法脊骨神經科醫師執業。而醫師在就讀脊骨神經科醫學院博士學位後醫期間必須通過 National Board Examinations Part I， Part II， Part III 的國家考試，始可報考各州所舉行之考試 ( State Board Examination )，通過之後，可註冊為脊骨神經科醫師 ( Doctor of Chiropractic, 簡稱 DC )，具此州之行醫執照，准許開業行醫。

三、脊骨神經科醫師之醫療範圍

1. 體檢、臨床檢查及診斷 ( Physical examination, History taking, Diagnosis, etc. )。
2. 合法操作 X 光 (具執照) 並監督放射科技術員 ( X-ray supervisor and operator )。
3. 臨床檢驗處方。如驗血、驗尿、核磁共振 (Magnetic Resonance Imaging)、斷層掃瞄 ( Computed Tomography )、心電圖 ( ECG )、肌電圖 ( EMG )、神經傳導 ( NCV )、超音波掃瞄 ( Diagnostic ultrasound )、都卜勒超音波 ( Doppler ultrasound )、Thermography、Videofluoroscopy、Radionuclide bone scan 等檢查。
4. 處方及操作物理治療及復健醫學。
5. 脊椎 / 肢體矯正。
6. 營養建議及食療。
7. 殘障鑑定及證明
